ANS-Online Colocation Request for Service (XLS)
To be used when requesting additional colocation services such as additional rack space, IP addresses, bandwidth, power or backup.

Consent and Waiver Form (PDF)
To be completed when Customer engages ANS to ship equipment from the data center to a Customer-specified location. Customer must send a completed form, as well as the Property Removal Authorization Form, to support@anscorp.com.

Property Removal Authorization Form (PDF)
No equipment can be removed from the Site without a signed Property Removal Authorization form, whether the equipment is shipped by ANS or removed physically by the customer. This form is a receipt, acknowledging removal of packages and assuming liability for packages assumed to be in good condition.
